The $500 Survey

Thank you for taking the time to fill our anonymous survey. Please only fill in the survey and entry ballot one (1) time (per household) between June 19 and 27, 2008 and you'll be entered into our random ballot draw for a five hundred dollar ($500.00) Carlingwood Gift Card. In the event that the drawn ballot belongs to someone under the age of eighteen (18) years the prize will be awarded to his/her parent or legal guardian. Present employees, or contracted employees, of Carlingwood Merchants, Management, Fenix Solutions and 20 Vic - and those with whom they are domiciled - are ineligible to win in this contest.
